How to Apply for Health Care Coverage: Medicaid, Healthy Michigan Plan, MIChild For Adults, Pregnant Women, Children, Families

The Department of Health and Human Services (DHHS) offers several ways to apply for health care coverage. Online applications are encouraged and will be processed faster than paper. After completing the application, you will be notified of the programs you and your family members are eligible for. See below for ways to apply and receive help.

 

  1. Apply online at home at www.michigan.gov/mibridges
    • Find more information on types of programs at www.michigan.gov/mdhhs and click Assistance Programs
    • Call the MiBridges HelpDesk for MiBridges application assistance 1-844-799-9876 (also in Spanish and Arabic), 8:00 am-5:00 pm, Monday - Friday
       
  2. Apply online at the DHHS offices at their kiosks:

  3. For in-person application assistance – Find a Navigator
    • Go to www.michigan.gov/mibridges and Connect with a Navigator (Search for nearby Navigator by entering your address)
    • Macomb County Health Department has a Navigator (English or Spanish) 586-469-5520
       
  4. Apply with the paper application which you can download at www.michigan.gov/mdhhs
    • Click on Doing Business with MDHHS, click Forms and Applications, click Assistance Application MDHHS-1171, click MDHHS-1171 Assistance Application and Program Supplement

Important Medicaid Phone Numbers After Enrollment

  1. Michigan Enrolls - to enroll in or choose a health plan 1-888-367-6557 (1- 888-ENROLLS) or 1-800-975-7630
     
  2. Beneficiary Helpline – problems with or qu estions about benefits; and to request a new MIHealth card 1-800-642-3195
     
  3. ModivCare Solutions (previously LogistiCare Solutions) – for non-emergency transportation to medical appointments 1-866-569-1902 for those with MI Medicaid. Those with Medicaid Health Plans must contact their health plans for transportation.
